Option 1

• Fill out the form
• Submit your information
• Pay online - fast, secure, easy!

Option 2

• Print out the application form (pdf)
• Fill it out, including the Medical Waiver portion
• Mail it to us with along with your payment

Refunds:
Prior to July 1st, there will be a $50.00 handling fee for all refunds. After July 1st, no refunds are available due to advance planning and operational/administration costs.

Medical Waiver Form:
A medical waiver (pdf) must be completed and signed by the athlete and parent/guardian. No camper will participate without a signed medical waiver (pdf).
