Option 1

• Fill out the form
• Submit your information
• Pay online - fast, secure, easy!

Option 2

• Print out the application form (pdf)
• Fill it out, including the Medical Waiver portion
• Mail it to us with along with your payment

Refunds:
Only Refund requests made before March 1st will be granted.

Medical Waiver Form:
The medical waiver portion on the application form (pdf) must be completed and signed by the athlete and parent/guardian. No camper will participate without a signed medical waiver.
